汇丰中国青少年高尔夫球公开赛(以下简称HCJGO)是由中高协汇丰青少年高尔夫计划委员会(以下简称“组织者”)组织管理的。考虑到“组织者”致力于对青少年高尔夫以及将于2011年1月18日-23日(包括行程日)在中国深圳正中高尔夫球会举行的HCJGO赛事的支持,本免责声明书的签署人,_______球员的法定监护人,在此免除“组织者”及其董事、代理、陪护人员、雇员、成员、所有权人、继任人及受让人因以上指名的选手和/或签字人在选手参加HCJGO的途中或赛事期间所遭受的任何损失、财产毁损或身体伤害(包括死亡)而产生或相关的一切责任、索赔、权利主张或诉讼请求,无论何种性质,且签署人进一步同意就“组织者”因上述 “选手”而发生或承担的所有的责任、损失、赔偿金、合理的律师费用、辩护费用、其他开销、裁决请求,向“组织者“予以赔偿。
签署人,作为上述命名选手的法定代理人,并且已充分了解到在参加高尔夫赛事及相关活动的途中及赛事过程中可能存在的危险和风险,在此自愿选择给予他们的书面批准。签署人作为上述命名选手的代理人在此自愿承担起与选手和/或签署人可能经受的所有损失、财产损毁或人身伤害(包括死亡)相关的一切风险。
此免责声明对球员及签署人的分销商、继承人、近亲、个人代表、遗嘱执行人、管理员、继任人和受让人亦具有法律约束力。
此条款将于以上提及的日程(即球员旅行并参加HCJGO赛事及其相关活动的所有日期)生效并具有完全的法律效力。
在签署此条款过程中,每位签名人在此确认并声明:
a) 他/她已阅读以上提到的免责声明,理解并自愿签署此免责声明书。
b) 任何签署人,以法定监护人身份签字的,应是以下签名球员的真实法定监护人。且每位签名人需超过18周岁并身心健全,同时在以下签名球员了解本免责声明内容并许可的前提下签署。每位签署人充分理解到此免责声明书涉及以下签名球员和签署人的一切主张和索赔权。

中高协-汇丰中国青少年高尔夫发展计划
参赛选手行为准则
(2010年1月1日开始施行)
为规范选手的比赛行为,培养选手的高尔夫球礼仪,营造健康的比赛氛围,保证正常的比赛秩序,由中高协-汇丰中国青少年高尔夫发展计划委员会(简称“委员会”)制定本准则。本准则经中国高尔夫球协会批准,由委员会指定的赛事组织委员会(简称“组委会”)监督执行,适用于每位获准参加委员会所举办青少年高尔夫球赛事的选手。
第一条 选手应遵守由英国圣安德鲁斯皇家与古老高尔夫球俱乐部(R&A)制定,并由中国高尔夫球协会批准的《高尔夫球规则》,也应遵守由组委会制定的当地规则以及其他规定。
第二条 任何在比赛中产生的争议,均可请求裁判进行裁决,选手应尊重裁判的裁决。如选手对裁判的裁决有疑义,可向裁判长及组委会提出申诉,由裁判长及组委会根据申诉流程做出最终裁定。选手应及时就疑义问题提请裁判裁决,否则后果由选手自负。一组选手全部提交记分卡后,不再接受申诉。
第三条 选手应遵守《高尔夫球规则》第一章所规定的礼仪规范。在单场比赛中,裁判有权对违反高尔夫礼仪的行为进行判罚,第一次将被提出口头警告,第二次将被罚一杆,第三次将被取消本场比赛资格。
第四条 选手应对比赛场内的任何人给予尊重,在比赛中不得对任何人做出侮辱性或攻击性的语言及行为,违者将被取消本场比赛资格。
第五条 选手应在规定的时间内完成比赛,不得以任何理由故意拖延比赛,裁判有权对无故拖延比赛的行为进行判罚,第一次将被提出口头警告,第二次将被罚一杆,第三次将被取消本场比赛资格。
第六条 选手应爱护球场及组委会的设施及财物,如有损坏应由选手或选手监护人照价进行赔偿,故意损坏球场设施及组委会财产的选手,将被取消本场比赛资格。
第七条 选手应穿着组委会指定的衣帽参赛,选手有以下且不限于以下情况者严禁参赛:不穿着组委会指定衣帽的;穿着背心、无领T恤衫、牛仔裤、短于膝盖的短裤、高跟鞋的;暴露纹身图案的;衣冠不整,歪带球帽及其它违反高尔夫球礼仪不符的服饰等。组委会将根据比赛实际发生情况调整严禁参加比赛行为的内容。
第八条 选手应按组委会规定参加全部比赛及组委会安排的活动,如选手因故不参加某项活动,需事先向组委会请假并获得允许,如选手中途退出比赛,需告知裁判,并将被取消本场比赛资格。所有已报到的选手必须参加选手会议,在一个年度内累计两次报到后不参加选手会议或迟到超过十五分钟的,将被取消一次参赛资格。报到时须由本人亲自到场,不得由他人代行报到。
第九条 选手如发现任何人有违反《高尔夫球规则》、当地规则、组委会各项规定或其他任何妨碍比赛正常进行的行为,均可告知并制止违规人,或向裁判及组委会检举,但不得自行处罚。
第十条 所有被取消比赛资格的选手,须立即跟随组委会工作人员离开比赛场地。对违反组委会规定的个人,组委会有权视情节轻重给予处罚,包括但不限于:取消本场比赛资格、取消参加委员会举办其他比赛的报名资格、向组委员提交书面检讨等。
第十一条 任何由选手行为所造成的法律后果,由当事人本人或其监护人承担全部法律责任,与组委会无关。
第十二条 汇丰全国青少年高尔夫球冠军赛组委会保留对本准则的最终解释权。

参赛选手陪同人员须知
(2010年1月1日开始施行)
为保证选手顺利进行比赛,维护比赛的正常秩序,由中高协-汇丰中国青少年高尔夫发展计划委员会(简称“委员会”)制定本须知。本须知经中国高尔夫球协会批准,由委员会指定的赛事组织委员会(简称“组委会”)监督执行,适用于每位获准参加委员会所举办青少年高尔夫球赛事的选手家属、亲友、教练、领队或其他陪同人员。
第一条 选手陪同人员不得以任何形式或任何理由干扰比赛的正常进行。
第二条 选手陪同人员应督促选手遵守《高尔夫规则》、比赛当地规则以及组委会的各项规定。
第三条 选手陪同人员应尊重组委会及裁判,服从裁判的裁决及组委会的决定。
第四条 选手陪同人员应遵守组委会关于选手陪同人员不得下场的规定,须在组委会规定的范围内观看比赛,任何选手陪同人员不得私自到规定范围以外观看比赛。
第五条 选手陪同人员应对比赛场内的任何人给予尊重,在比赛中不得对任何人做出侮辱性或攻击性的语言及行为。
第六条 选手陪同人员应爱护球场设施、组委会财物,如有损坏须照价赔偿。
第七条 选手陪同人员如发现任何人有违反《高尔夫球规则》、比赛当地规则、组委会规定及其他任何妨碍比赛正常进行的行为,都有权告知并制止违规当事人,或向裁判或组委会检举,但不得自行处罚。
第八条 选手陪同人员在陪同选手参赛期间,有如下行为的,组委会有权取消相应选手的参赛资格,并要求选手及选手陪同人员离开比赛场地。组委会还可视情节轻重,要求选手陪同人员提交手写书面检讨,或不再接受选手本人的报名。
包括但不限于:任何违法行为;对任何人作出殴打、辱骂、恐吓等攻击性或侮辱性行为;故意损坏球场设施、组委会财物或其他人的财物;抢夺、藏匿、故意损坏任何比赛用品或选手球具;在比赛进行过程中到规定范围以外观看比赛;故意拖延比赛正常进行;欺骗或教唆选手欺骗对手、裁判或组委会;扰乱赛场正常秩序等。组委会将根据比赛实际发生情况调整“违规”行为的内容。
第九条 任何由选手陪同人员行为所造成的法律后果,由当事人本人承担全部法律责任,与组委会无关。
第十条 汇丰全国青少年高尔夫球冠军赛组委会保留对本准则的最终解释权。

CGA-HSBC China Junior Golf Program’s
Instructions for Players’ Chaperones
(Start date: 1st Jan, 2010)
To ensure the successful implementation of tournament proceedings, the following regulations will be instituted by the committee of the CGA-HSBC China Junior Golf Program. These regulations have been approved by the CGA and will be executed by the tournament committee (named committee below), and should be followed by all relatives, friends, coaches, and other chaperones of any player participating in golf tournaments organized by the committee.
1. Players’ chaperones may not interrupt or delay the competition in any way or for any reason.
2. Chaperones should ensure players follow both the golf rules as well as the local rules set by the committee.
3. Chaperones should respect the committee and its referees, and will agree to honor referees’ and the committee’s decisions.
4. Chaperones may not go onto the golf course and are restricted to observing tournament proceedings from an appointed viewing area. Chaperones are not allowed to go beyond this area.
5. Chaperones should show respect to everyone on-course; rude and offensive behavior will not be tolerated.
6. Chaperones should respect the golf course’s facilities and possessions, eg. agree to compensate the committee for any damages that may be incurred due to chaperone negligence.
7. If a chaperone comes upon a violation of golf rules, committee regulations, he/she should not take it upon themselves to mitigate or resolve the violation; rather, they should notify a referee or committee official immediately.
8. The Committee reserves the right to disqualify players and will ask players and their chaperones to leave the tournament grounds if any parties incur the following acts:
Including but not limited to: any illegal acts; incidents of assault, verbal abuse, threatening or insulting acts; damaging golf course facilities, committee belongings or others’ property intentionally; marauding, skulking, damaging tournament appliances or other players’ golf equipment; watching competition from beyond the appointed viewing area; intentional delay of the tournament; deceiving or enticing other players to cheat or break the competition’s rules; disturbing the tournament, etc. The committee reserves the right to define the legality and severity of infractions according to the outcome of events. It also has the right to request (from either said player or chaperone) written statements admitting full legal responsibility for said infractions, with the alternative being banishment from the program.
9. Chaperones will bear all legal liabilities if said parties act against the law; the committee bears no responsibilities for the transgressions of these individuals.
10. The Committee of HSBC National Junior Golf Championship will reserve the rights to final interpretation of the above guidelines.

Code of Conduct for Players
(Starts from 1st Jan, 2010)
In order to standardize players’ conduct, cultivate golf etiquette, create a healthy competition environment, and make sure the tournament proceeds successfully, this code of conduct has been put forth by committee of CGA-HSBC China Junior Golf Program. These rules have been approved by the CGA and will be executed by the tournament committee (named committee below), and each player approved to play in golf tournaments overseen by the committee should abide by them.
1. Players should abide by the golf rules from The Rules of Golf from the R&A and approved by CGA, as well as to the local rules set by the committee
2. Any disputes that occur during the tournament can be subject to ruling from the referees; players must agree to respect the referees’ decisions. Players can, however, submit an appeal to the chief referee and committee, who will then subsequently make a final ruling according to the appeals procedure. Should any incident occur; players should promptly request for a ruling from a referee or else be prepared to bear all the consequences otherwise. Appeals will not be considered if not requested before all players submit their final scorecards.
3. Player should abide to the etiquette described in the first chapter of the Golf Rules. Referees have the right to penalize players who do not adhere to regulations as follows; first offence—oral warning, second offense—one stroke penalty, third offense—disqualification.
4. Players should show respect to everyone on-course, and refrain from being rude, insulting and offensive.
5. Players should resolve play within the allotted time, and should not delay competition. Referees have the right to penalize players who delay competition for no reason; first offense—oral warning, second offense—one stroke penalty, third offense—disqualification.
6. Players should respect the golf course facilities and committee property. Players (including their guardians) are responsible for compensation if any damage is incurred. Players who damage golf course facilities and committee property will be disqualified.
7. Players should wear apparel approved by the committee. Players who commit (including but not limited to) the following acts will not be accepted: not wearing apparel appointed by committee; wearing vests, non-collared T-shirts, jeans, pants above the knees, high heels; exposing tattoos; not properly dressed, (cap not in proper order and other acts against golf etiquette) etc. The committee may adjust the above in accordance with tournament circumstances.
8. Players should attend the entire tournament and all related activities arranged for by the committee. Players who cannot attend an activity should ask for a leave of absence from the committee in advance, otherwise, they will be reported to the referees and be disqualified. All players who have registered should attend the players’ meetings. Players who have been absent twice in one
year or have been late for over 15 minutes twice in one year will be stripped of their eligibility one time. Players should register in person, and not send a substitute to register for them.
9. If anyone is found to be in violation of the Golf Rules, local rules, committee regulations, or anything that might disrupt the tournament, players should inform the individual and/or report it to referees or the committee, rather than attempting to mitigate the violation themselves.
10. Players who have been disqualified should follow committee officials and leave the grounds immediately. The committee reserves the right to issue penalties against individuals who do not conform to these rules, including, but not limited to disqualification, subsequent expulsion from all committee-run events. A written apology would have to be submitted to the committee for review to regain eligibility for entering other tournaments.
11. Players and guardians will be responsible for all legal liabilities if found to have acted against the law; the committee is not liable for the transgressions of these individuals.
12. The Committee of HSBC National Junior Golf Championship will reserve the rights to final interpretation of the above guidelines.

HSBC China Junior Golf Open (hereafter called HCJGO), is organized by the CGA-HSBC China Golf development Committee (hereafter called Organizers). In consideration for ORGANIZERS' involvement in supporting junior golf and HCJGO to be held in Genzon Golf Club,Shenzhen, China between the dates of Jan 18 and Jan 23, 2011, inclusive of travel days, the undersigned, being the legal and acting guardian of and on behalf of the player name , release and hold harmless ORGANIZERS, their directors, agents, escorts, employee, members, owners, successors and assignees of any and all liability, claims, demands, actions and causes of action whatsoever, arising out of or related to any loss, property damage or personal injury, including death, that may be sustained by the player named above and/or the undersigned while the player is in travel to, or participating in HCJGO. and further agrees to indemnify ORGANIZERS against all liability, loss, damages, reasonable attorney's fees, defense costs, expenses, judgment claims and demands which the ORGANIZERS may incur or become responsible for by reason of the above mentioned player.
The undersigned, being duly aware of the risks and hazards in travel to, and participating in a golf tournament and related activities, and acting for the player name above, hereby elect voluntarily to grant their written permission. The undersigned acting for the player name above, hereby voluntarily assume all risks of loss, property damage or personal injury, including death, that may be sustained by the player and/or the undersigned.
This RELEASE shall be binding upon the distributers, heirs, next of kin, personal representatives, executors and administrators, successors and assignees of the player and of the undersigned.
This RELEASE shall be in full force and effect, inclusive of the days as above indicated, which the player is involved in travel, participation, or related activities as to the HCJGO
In signing this RELEASE, each of the undersigned hereby acknowledges and represents:
a) That h/she has read the foregoing RELEASE, understands it, and signs it voluntarily.
b) That any undersigned, signing as 'legal guardian', is in fact a true legal guardian of the named below, each of the undersigned is over the age of eighteen years and of sound mind and has signed this RELEASE with the knowledge and consent of the player who has signed below, and each of the undersigned understands that this RELEASE relates to any and all claims of the player named below and the undersigned.
